INTRODUCTION The private health insurance system in the U.S. has been erected on a foundation of tax incentives that promote employment-based coverage over the individual purchase of insurance. In 2006, persons taking advantage of tax breaks for health insurance will save about $150 billion in federal and state income taxes and an additional $75 billion in payroll tax contributions (Sheils and Haught, 2004; Sheils, 2006). (1) Most Americans--some 174 million people, or about 70 percent of those with insurance--are covered by employment-based health insurance (DeNavas-Walt, Proctor and Lee, 2005). Included in that count are about 12 million seniors covered by Medicare who also have supplementary retiree coverage through a former employer.
